Needs Assessment & Service Co-ordination
Counties Manukau DHB has a contract to provide a service for those 65
years and over who have a disability.
The
MOH definition of disability is:
"A person with a disability is a person who has been identified as
having a physical, psychiatric, intellectual, sensory or age related
disability (or combination of these) which is likely to continue for a
minimum of six months and result in a reduction of independent function
to the extent that ongoing support is required"
The
aim is to support the client and the care giver to remain in the
community if possible and to maintain their quality of life. If this is
not possible we arrange suitable long-term care. The service is
contracted to assess the client and to identify the areas of need. The
assessment includes the needs of the caregiver/whaanau/family.
The service plan can include support from a range of individuals, groups
and organisations.
The
service arranges specialist assessments e.g. Geriatrician, Occupational
Therapist, Physiotherapist, Social Worker. The identified needs are
prioritised, as with limited resources it is not always possible to meet
all the needs.
Service Co-ordination involves informing the client of the options and
arranging services.
